The Disadvantages of a One Size Fits All Education

The public schools that most kids attend have several disadvantages compared to homeschools. Plastic Surgery Revision of Tracheostomy Scars

Tracheostomies frequently result in unsightly neck scars. Their prominent neck location and the temporary indwelling breathing tube commonly leaves a depressed neck scar after its removal. It often looks like a belly button with the skin turned inward. This is known as a tracheal tug although it is the lack of underlying soft tissue that makes it look this way.

An undesired looking scar from a tracheostomy can undergo correction within months after a breathing tube has been removed if so desired. Historically, tracheostomy revisions have been done when the scar is more mature. (greater than six months after tube removal) It is not necessary to wait this long however. The plastic surgery techniques used are not really affected by how mature the tracheostomy scar is. And, for many patients, they would like to eliminate as soon as possible the physical and psychological marks of the experience of why the tracheostomy was there in the first place.

To get a good outcome from a tracheostomy scar revision, there are three basic concepts that must be achieved surgically. First, the turned in skin edges must be released from the deeper tissues and be completely freed up. Secondly, this release creates a real tissue volume defect between the skin and the trachea which must be filled in. Lastly, closing the skin must create a fine line scar that lies in a horizontal direction along a natural neck skin fold. By far, filling in the lost tissue is the hardest one to achieve but it absolutely necessary if one does not want the final result to have any indentation. Filling in the missing tissue can be done in a variety of ways. If the tracheostomy scar is not that deep and fairly shallow, the surrounding skin edges can be used through a technique known as edge de-epithelization. The thinned skin edges are then turned down for a little tissue fill and the full-thickness skin edges closed over it. For tracheostomy scars that are significantly indented, however, more volume is required. I prefer using dermal-fat grafts which can be quite thick if desired, up to 1 cm. A donor site is needed to harvest it and this will leave a scar elsewhere on the body. However, if one has a scar from a previous surgery elsewhere that is from a favorable area that has some fat thickness, then this should strongly considered. Otherwise, allogeneic dermal grafts (from cadaver skin) can be used which is an off-the-shelf product.

Tracheostomy scar revision is a fairly simple outpatient procedure. All sutures are placed under the skin so there is none to remove. A fine line red scar will exist for awhile afterwards (months) but this will eventually fade into a near imperceptible pencil line thin scar. Again the key to a successful tracheostomy scar revision is to completely resolve the skin tethering to the trachea and replace any missing tissue.

Knock Out Your Tension - Try Boxing Classes!

In the 1990s, martial arts champion Billy Blanks released a series of videotapes that combined martial arts (Tae-Kwon-Do) with boxing for a unique activity that promised to deliver exceptional health and fitness benefits. Calling his method Tae Bo (pronounced 'tie-bo'), Blanks punched, kicked, and sparred his way into the living rooms-and the fitness routines-of millions of Americans. Suddenly, boxing was not just a bloody contact sport, it was a fitness activity that brought massive health benefits to its participants.

Although boxing for fitness was hardly a new concept, Tae Bo glamorized it and made it more accessible to the everyday exerciser seeking to introduce a fun and entertaining aspect into their exercise routines. Consequently, many fitness centers today offer boxing classes to their members who seek to achieve peak physical conditioning in a short amount of time. But fitness-boxing routines do more than simply firm the body. Boxing has proven to be an effective way to relieve the tension that results from the stresses of modern society.

Exercise and Stress-Relief

Many studies have shown that physical exercise is an effective stress-reliever. One such study was conducted at the University of Colorado at Boulder in 1999. This 4-part study was designed with the goal of, according to Assistant Professor Monika Fleshner, understanding "how regular, moderate, physical activity alters the stress response by examining the entire system, from the brain to the individual cells."

To that end, the researchers conducted four separate but related experiments on mice. Each of these experiments yielded convincing proof of the stress-reducing effects of exercise and, by extension, of boxing classes.

For this study, a group of physically active rats (those who maintained a regular routine of wheel-running for 4 weeks) were compared to a group of sedentary rats after being exposed to 90-minutes of stress. In three of these experiments (the fourth one did not concern stress reactions), the physically active rats showed a marked difference in their reactions to stress as compared with the sedentary rats.

- Experiment 1. The physically active rats produced a lower amount of a stress-induced protein.

- Experiment 2. The physically active rats released less norepinephrine (a hormone that is similar to adrenaline).

- Experiment 3. The physically active rats were less likely to suffer the known negative physiological results of stress that include an increase of stress proteins, a suppression of cell division, and a decrease in cytokines (which causes a lowered immune response.)

The results of this and many other studies show that exercise is an effective stress-reliever that causes many physiological changes. Indeed, all it may take to knock out your tension, say experts, is to participate in boxing classes.

The Ideal Exercise
The ideal exercise routine combines aerobic and anaerobic activities, can be performed without undue risk of injury, increases calorie burn, and provides enough variety to avoid boredom. By this standard, boxing classes provide an excellent total workout for the body-and for the mind (via reduced tension and stress).

Boxing has been shown to increase the efficiency of the cardio respiratory and vascular systems, hone the performance of the central nervous system, and strengthen the musculoskeletal system.

Boxing Classes
Most boxing classes last for 60 minutes and, although the mix of exercises differs for each program (and for each fitness facility), one can expect the class to contain the following components:

Warm Up. This segment at the beginning of the class lasts for 5-10 minutes and is designed to warm the muscles and tendons, preparing them for the more strenuous portion.

Conditioning. The warm up typically transitions into several boxing moves that are designed to condition the body. These exercises provide anaerobic (strength) conditioning together with an aerobic component to increase the heartbeat. The conditioning segment of boxing classes typically lasts for 10 minutes.

Technique. A few new boxing techniques are introduced at each session. In the beginning, these techniques will be simple movements such as punches and jabs. After these are mastered, more complicated (and challenging) combinations of boxing movements will be added to the program. The training in each specific boxing technique lasts for approximately 5 minutes.

Hitting Drills. The prime focus of most boxing classes are the "hitting drills" which revolve around a timed series of punching and kickboxing of stationary objects such as heavy bags. The objects to be hit will be positioned as part of a circuit that the "boxer" will hit or kick (employing proper boxing technique) for approximately two to three minutes. Between each station, the boxer will rest for 1 minute before going to the next station for a continued two to three minutes of punching and kicking. (There may also be standard exercise machines within the circuit to provide a more complete workout.) This routine continues until the boxer has worked all stations in the circuit. This part of the boxing class, which lasts for approximately 35 minutes generally leaves the participants feeling energized and refreshed.

Cool Down. This last part of the class focuses upon gradually slowing the heart rate while practicing gentle stretching exercises that will help to prevent or reduce later onset of muscle soreness and joint stiffness.

Boxing for Everyone
Although boxing can be a very intense form of exercise, it can also be modified to fit anyone's fitness level. To prevent injury, Felix Pagan, the manager of the LA Boxing Gym in Rockville, Maryland advises the individual to work on perfecting proper boxing form as well as building muscle and increasing cardiovascular strength. "When your form is the way it should be," Pagan says, "it helps you execute [your moves] and your exercises are more effective." Being tongue tied is more than just not knowing what to say. It is an actual medical diagnosis (ankyloglossia) that is not that uncommon. (estimates are one in one to two thousand people) In my Indianapolis plastic surgery practice, I have seen lots of them over the years. It is a condition inside the mouth where the tip of the tongue is tethered down to the floor of the mouth. It is marked by an inability to move the tongue beyond the front teeth.....or just barely past them. When there is an attempt to stick the tongue out, a V-shaped notch appears at the tip. Technically, it occurs when the lingual frenulum (on the underside of the tongue) is either too short or anteriorly placed , restricting the mobility of the tongue.

Ankyloglossia occurs as a result of a developmental aberration in utero. Early in fetal development, the tongue is attached to the floor of the mouth. Like how fingers become separate from the initial fetal webbed hand (planned cell death and atrophy), the tongue and the floor of the mouth separate leaving a residual attachment known as the frenulum. Tongue-tie results when this frenulum is short and thus limit the movement of the tongue. This is a condition which can be seen at birth if one looks carefully enough. But it becomes very apparent when the front teeth have erupted. Physical exam will easily demonstrate the short or anteriorly placed lingual frenulum. While the tongue continues to grow until about four years of age, one does not outgrow a short frenulum. Once present, it remains so.

Tongue-tie most commonly appears as a partial restriction and rarely as a complete fusion. There is considerable controversy as to what functional problems that it may or may not cause. It has been implicated in speech defects, breastfeeding difficulties, and a source of dental problems. While I have seen and treated many tongue-ties in babies and young children, the few I have seen in adults did not appear to be causing any significant problems. The main reason tongue-ties are treated is for better tongue mobility which has a cosmetic benefit and probably some minor functional improvement as well.

The main reason to treat tongue-tie, in my opinion, is the sheer simplicity of doing it without postoperative complications or relapse. One can have a lot of debate as to whether it is medically necessary, but if you can eliminate the problems with a very simple and quick procedure, that debate takes on less significance.

Tongue-tie is released through a procedure known as a frenuloplasty or frenectomy. The frenulum is incised near its midportion until the tongue is fully released. Then it is sewn together with small resorbable sutures creating a long unrestricted frenulum. In babies and children this requires a general anesthetic. In adults, it can be done under local anesthesia in the office.

Indianapolis Semi-Pro Football Team Wins National Championship

The Hoosier Hurricanes invaded Homestead, Florida for last weekend's 4th annual Minor League Football News (MLFN) Super Football Weekend strong winded and left with reign.

In a game dominated by defense, the Hurricanes hammered the Carolina Express 41-0 to win MLFN's AA National Championship.

INDIANAPOLIS

"To reach this point so quickly is incredible," said Hurricanes head coach John Starlin. "I really feel like a proud father to all of those guys."

While the 'Canes' offense was strong, the play of their defense was dominant. The 'Canes forced eight turnovers and controlled the Express' offense, which towered over them in size and power. Early in the contest, it looked like the Express were going to use their Power I offensive attack to run right down the throat of the 'Canes' defense. When the Express were forced to throw, however, the speedy Hurricane secondary was able to take the game away. The first of four Express interceptions was thrown on that opening drive, and the Hurricanes never looked back.

"They were a tough team to scout, but I really think they underestimated our speed," said Starlin. "Other coaches from their league were able to help me out as far as scouting, but I knew as solid as our defense was that we could play with anybody."

Staying with the defensive theme of the game, linebacker Deon Smith was awarded the game's MVP for his 12 tackle performance. He also intercepted a pass and forced a fumble.

The AA national title was the perfect end to a season that was already storybook for the Indianapolis squad, founded by co-owners Starlin, David Day, and NFL wide receiver Reggie Wayne. In their first year of existence, the Hurricanes were awarded Best New Team in the Midwest by MLFN after finishing 13-1 in the Ohio Valley Football League (OVFL). Their OVFL season finished with a Super Bowl victory over the Charlotte (MI) Roughriders, 21-0.

"This has really been a dream season for us," said Starlin.

Starlin's players have also won numerous accolades from MLFN. Four 'Canes were honored with Midwest All-American status: Derrick Ellis was an honorable mention Midwest All-American at running back, wide receiver Scott Penick made second team Midwest All-American, and Corey Crumpton and Charles Avant were first and second team at defensive back, respectively.

Ellis, a former "Mr. Indiana" while playing running back in high school, was also named Running Back of the Year in the Midwest Region by MLFN.

The Hurricanes will return to the OVFL next season and compete to become the first ever back-to-back champion in league history, a feat no team has accomplished in 40 years of competition.

"We are trying to develop something special here," said Starlin.

It certainly looks like the Hurricanes are off to a roaring start.

Negligence Per Se

Typically, for a personal injury case to be proven, it must be shown that the defendant was engaging in some form of negligent behavior. This requirement exists to protect people from liability in the event of freak accidents which are completely unforeseeable. If a light pole is struck by lightning, causing it to collapse and strike a pedestrian, it is most likely that the property owner will not be considered liable for this injury, as no one could have predicted the chance that a pole would be struck by lightning and would subsequently strike a pedestrian.

The claim of negligence is based on the notion that we have a moral and legal duty to not expose others to risk to which we have created and to which they have not consented. If we are engaging in actions that carry with them an inherent amount of risk, we have a duty to minimize the amount of risk that we expose to others. If we fail to do so, then it is said that we are behaving negligently, and if someone is then injured as a result of our actions, we may be liable for those injuries.

Thus, if the criminal courts find someone guilty of violating a statute designed to keep the public safe and free from unnecessary risk, they are considered automatically negligent in civil courts.

One of the most noteworthy methods that is currently marketed and touted is laser liposuction, or more accurately, laser-assisted liposuction. As opposed to the traditional technique of cannula insertion, eating up the fat tissue, and then sucking it out, the mechanical beat-up is replaced by laser energy. A laser fiber is inside the liposuction cannula which delivers heat to the fat cells, thus breaking them down inito a more liquid state making it easier to be sucked up the cannula. In theory, the heat from the laser also causes the overlying skin to tighten.

As of this date, there are three such laser-assisted liposuction systems available in the U.S. They are under the marketed trade names of SMARTLIPO (Cynosure), COOLLIPO (CoolTouch), and PROLIPO (Sctiton). All of them tout more effective fat removal, less bruising and swelling, a faster recovery, and skin tightening.

The real question is....are they really better than traditional liposuction...and are there increased costs over traditional liposuction worth it? (someone has to pay for the equipment purchase...and that is you the patient!) To date, they remain tremendously appealing, particularly to the general public. Historically, any treatment associated with the word, laser, always causes mass patient allure. No clinical trials have yet been reported, that I have seen, that have ever directly compared any laser-assisted vs traditional liposuction procedure. Nor has any large clinical trial been done that even compares two differently-treated patient groups, no matter how impossible it really is to 'match' patients from different groups.

Much like ultrasonic-assisted liposuction when it was introduced 10 years ago, laser-assisted liposuction may be a popular trend that, in the end, will fare no better. It will undoubtedly have its place in liposuction options and armamentarium but I doubt if it is magic and offers a quantum leap in patient outcomes and recovery times. It may offer some improvement in fat removal efficiency, but I am truly dubious of overall better outcomes, a much better and faster recovery, and skin tightening (of which I remain the most skeptical)

I am all for any form of advancement in liposuction, which is a fairly traumatic procedure on the tissues. While effective, liposuction is still one of the most 'brutal' procedures that we as plastic surgeons do. Is laser-assted liposuction a real improvement...ask me again in 5 years!

Freakonomics - A Journey on Challenging Conventional Wisdom Through Economics

Reading a book about economics is probably good for anyone in today's business world or for myself, a small business coach. Yet, the authors, Steven D. Levitt and Stephen J. Dubner, of Freakonomics provided their readers a lot more than just numbers.

The first hook that I received was how the authors defined morality and economics. Not, I am not going to share those definitions with you. You need to buy the book or check the book out at the local library.

Then the authors proceeded to connect seemingly unrelated events through 6 chapters from What Do Schoolteachers and Sumo Wrestlers Have in Common? to What Makes a Perfect Parent? using the science of measurement. These are not your standard economic topics by conventional wisdom. Their efforts reminded me of the Connections television series hosted by James Burke (science historian) that debuted in the late 1970's.

What the authors accomplished for me was:

To confirm through some unique examples that we as human beings have a tendency to confuse cause and effect To look beyond the accepted conventional wisdom with a different perspective by asking the unasked questions

In far warning, part of this book might be viewed quite negatively by some readers. The authors did their best to balance their findings against anticipated moral outrage.

Again, conventional wisdom many times has us throwing out the baby with the bath water. Levitt and Dubner are asking you to question what you truly know against what you have been told. You may not agree with their findings, but the process of open and honest questioning should be the conventional wisdom within every individual.

Rift Classes Guide - Which One To Play?

One of the basic things you should know before starting to play Rift is which class you want to play. In Rift these class archetypes are called Callings, and there are four different ones: Cleric, Mage, Rogue and Warrior.

Clerics are the primary healers in Rift: Planes of Telara and are absolutely crucial to have in instances, PvP and rifts. Without a healer other classes will have trouble surviving and your role is to keep everyone else -- including yourself of course alive for as long as possible. The best part about clerics in Rift is that they are unlike most healers in other similar games: clerics are more than capable of defending themselves if they are attacked, and considering they wear mail armor they are a lot tougher to kill than rogues or mages. Playing a cleric can be very fun and rewarding, and I encourage you to give it a shot. The best part is that you will be able to find groups for dungeons and raids quite easily as there is always a shortage of healers. Mages are excellent ranged damage dealers who also excel in crowd control abilities. They are able to keep several opponents at a distance for quite awhile, giving other classes some much needed breathing room in hard encounters as well as PvP. Different mage souls have different sets of abilities and therefore different party roles so make sure you research each one before you pick one, as you might easily regret the decision later. Rogues are physical damage dealers. Some of them attack from afar and some from melee distance. For example a Marksman is specialized in ranged combat with bows, therefore can be nicely paired with a Beastmaster which has melee pets to soak up monster aggro and damage for them. Assassins and Blade Dancers for example are specialized in melee combat and quickly killing their target, preferably while staying under stuns and other disables is what they are best at. Finally Warriors are the main tanks in the game, meaning they take up monster aggro on themselves because of their high damage resistances. It enables other classes to do their job unhindered making warriors absolutely crucial to have in difficult encounters. Not all warriors are specialized in tanking though, as certain callings are more than capable of dealing damage as well. Much like clerics warriors can easily find groups for instances as there are always plenty of damage dealing classes, and never enough tanks or healers.

Whichever calling you decide to play in Rift I'm sure you'll have a lot of fun. Make sure you pick the right souls which have good synergy and you will quite easily be able to handle anything you encounter in this exciting game.

Throw your yoga DVD into the player and you can do at least most of what you did at class. Again, yoga is best done at a yoga studio with an instructor who can teach you if you are a beginner but doing it at home in between classes and when you can't make a class is a great way to keep to your fitness routine and goals. I have found the most successful people who really accomplish their New Years goals do it through yoga classes as opposed to all other types of fitness.

1895 William G. Morgan (1870-1942) invented volleyball, an Olympic sport since 1964, in Holyoke (Massachusetts).

1954 Flora "Flo" Hyman, an ex world-class player (1973-1984), was born on July 29, 1954 in Inglewood, California. This sportswoman was well-known for her fair play. She won two Norceca championships, one Olympic silver medal, one world bronze medal and other international titles.  

1960 Charles Frederick "Karch " Kiraly was born on November 3, 1960 in Jackson (Michigan). He has won three Olympic gold medals (indoor and beach volleyball).  

1964 Under the leadership of Ron Lang, the United States competed in the Olympic Games in Japan-the first Olympic Volleyball Tournament.  

1973 The U.S. men's squad earned the Norceca title in Tijuana (Mexico). In the final, they beat Cuba, Pan American champions and Olympic players, 3-0 (15-12, 15-11, 15-10).  

1974 Under the direction of Carl Mc Gown, head coach of the U.S men's volleyball squad from 1973 to 1976, the American team visited Moscow (Soviet Union) and played friendly matches against the USSR, Cuba and Japan.  

1977 At the Women's World Championships in Japan, the United States beat the USSR, world champion in 1973, 3-1.  

1978 The Cuban men's team toured in Ohio (Dayton), Indiana (Cincinnati & Muncia) , Michigan (Indianapolis Bay) and Illinois (Madison & Wheaton).The USA and Cuba played seven matches...  

1979 The United States women's team earned a silver medal in the Norceca championships and placed fourth in the Pan American Games in San Juan de Puerto Rico.  

1980 For political reasons, the American women squad could not trip to the Summer Olympics in the Soviet Union.  

1981 Doug Beal was named head coach of the U.S. men's volleyball squad.  

1982 The U.S. women's volleyball squad, under the direction of Arie Selinger, became the first North American team to win a world medal.  

1983 The female team gained the Norceca Tournament in Indianapolis. In August, it was runner-up at the Pan American Games in South America.  

1984 In Los Angeles, California (USA), the American men team, under the watchful eyes of Doug Beal, earned the Olympic title. America did not expect to win the global title. The best players were Karch Kiraly, Dusty Dvorak, Craig Buck, Paul Suderland, Aldis Berzins and Marc Waldie.  

1985 Rose Magers was one of the members of the World All-Star team. This Afro-American volleyball player was born on June 25, 1960 in Big Spring, Texas...The male team earned the Korea Cup. They defeated Japan, Mexico and South Korea.  

1987 The United States and Cuba played five friendly matches in Havana. The U.S. men's squad won three matches. Subsequently, they were Pan American champions.  

1995 The female squad became the third national team to win the World Grand Prix.  

1996 The First Olympic Beach Volleyball Tournament was held in Atlanta, Georgia (USA).Two local pairs earned the gold and silver medals in the men's tournament. America's Karch Kiraly, a former star volleyball player, and Kent Steffes became the first American pair to take an Olympic gold medal.  

2000 The American national beach volleyball team, Dan Blanton & Eric Fonoimoana, earned the global title at the Sydney Olympic Games. In the final, they defeated Jose Marco Melo and Ricardo Santos (Brazil).  

2005 Lang Ping became the first woman to be named head coach of the U.S. women's national team. Lang, best known as "Jenny", was born and grew up in the People's Republic of China. In 1982, she was considered the most valuable player in the world.  

2007 At the Norceca Tournament in Puerto Rico, the U.S. men's squad took the regional title and won the right to compete at the FIVB World Cup in Tokyo (Japan).This team finished fourth place in Japan.  

2008 The American volleyball team won two medals, gold and silver, at the Summer Olympics in Beijing (China). This Olympic sport became one of the most popular sports in the United States...

Aging Upper Lip Lines

One of the features of facial aging that bothers women the most is the development of upper lip lines. Whether they be small fine lines that are at the junction of the skin and upper lip or much deeper lines that extend vertically way up into the skin...women hate them...as they indicate a more aged mouth look and in more severe cases allows lipstick to bleed up into the lines. Many patients think it is due to smoking and chronic sun exposure (and clearly these can make a big contribution) but the reality is...it is a function of full your lips are and how thick your skin is.

This is why this is much more of a problem in Caucasians of northern European and English descent than it ever is in African-Americans, southern Europeans, or those of Mediterranean origins. As all natural wrinkles form perpendicular to the direction and action of the underlying muscles, vertical lip lines develop from the action of the circumferential orbicularis muscle that purses your mouth around a straw, cigarette, or puckers for that kiss.

While the diagnosis of this problem is easy, it's treatment is not. Since you can't cure the cause of the lines (thicken your skin or stop moving your mouth), I tell patients to think about improvement (but not elimination of the upper lip lines) and the need for maintenance treatments as we are not curing the root of the problem. Treatment choices are based on three achievable objectives; diminish the muscle movement (Botox), plump up the lips or fill the lip lines (injectable fillers), or 'sand' down the wrinkles. (dermabrasion or laser resurfacing). Any combination of two or more of these will produce a better result in most patients. While Botox can certainly weaken the muscle movement and cause less puckering, it must be done carefully so as not to create an unnatural lip movement with smiling.

For this reason, this is usually the last procedure I will do or will do it only in combination with everything else in the most severe cases. Injectable fillers are a good option if the patient can accept or wants a larger lip. If not, then dermabrasion or medium-depth laser resurfacing is the only other option. The best results that I usually see is when the upper lip is slightly enhanced with a filler and the upper lip is then laser resurfaced at a depth of 50 - 100 microns.

This is probably the best combination if, again, the patient can accept a larger upper lip. It heals within a week and can be done in the office under local anesthesia. Patients will usually have to repeat the procedure once a year for maintenance of good results. However, it is fair to say that upper lip lines defy one single permanent solution.

The Salvador Dali Museum is located on the campus of the University of South Florida St.Petersburg, and has the largest amount of works from the artist Salvador Dali outside of the European area. The collection of the artist's works at the Museum include nearly 100 oil paintings, lots of graphics and prints, watercolours and plenty of written material from the great artist.

The Children's Museum of Indianapolis is the largest children's Museum in the world and is located in Indianapolis City in the State of Indiana. The Museum was founded in 1925, with the current building created in 1976.
