The Gummy Smile - What Can Be Done About It?

In the ideal smile, the upper lip sits just at or above the top of the teeth. The general rule is that 1 to 2 millimeters of gum exposure with a maximal smile is aesthetically acceptable. When some people smile, however, a large gap becomes exposed between the lip and the upper teeth, exposing a lot of gum tissue and producing the so-called gummy smile. The gummy smile can be due to a variety of factors including a hyperactive muscle which lifts the upper lip too much, a thin upper lip which rolls under when smiling, gum tissue which covers too much of the upper teeth, and a long upper jaw. Careful examination of the smile, at rest and when smiling, can help identify which one(s) of these factors is contributing to the gummy smile problem.

The classic minimally invasive treatment is the excision of gum tissue also referred to as a gingivoplasty. The gingivoplasty is the removing of excess gum tissue from the top part of the teeth, making the tooth look longer and lessening the amount of gum tissue exposed between the lip and the teeth, One must be careful to not remove too much gum tissue which can expose the root portion of the tooth leading to chronic sensitivity. Occasionally dental veneers can be used as part of this procedure to help increase the amount of tooth show.

Other minimally invasive procedures include the injectable treatments of Botox and off-the-shelf fillers. Injecting Botox into the hyperactive muscles of the upper lip which help lift it will reduce how much the upper lip moves upward resulting in less of a gummy smile. Like all Botox injections, however, the result will last at best for four months. But it can be tremendously useful in someone who has a big wide smile movement. If the upper lip is also thin, injectable fillers such as Juvaderm or Perlane can be added to help bring down the vermilion border (lower edge of the upper lip) slightly. These injectable fillers will last longer than Botox, lasting up to six to months. In a few cases, I have done both at the same time which overall creates a better effect than either one alone.

Another option to reduce a 'high lip line' is an upper lip lowering procedure. This minor operation was initially introduced in the 1970s but has only recently become more popular. It is done with a local anesthetic and involves the re-attachment of the gum tissue under the lip (this does not involve any surgery on the lip that you see) at a level that is closer to the teeth. This results in not only lowering the lip line at rest but also limiting how much it moves upward when smiling. This procedure shortens the depth of the vestibule (space between the upper lip and gum tissue high up under the lip) to help bring the lip down somewhat.

In severe gummy smile cases, the problem is skeletal in nature with a long face syndrome. The upper jaw has simply grown down too much. In so doing, it carries the teeth way below the upper lip and a lot of gum tissue can be seen, often up to 5 to 8 millimeters too much. These patients also have difficulty in putting their lips together with a pseudo short upper lip appearance. Correction requires upper jaw surgery (LeFort I impaction) to remove a wedge of upper jaw bone and allow the visible tooth part of the upper jaw to 'go back up under the upper lip'.

The proper treatment of the gummy smile is based on a good evaluation to determine what anatomic problems is making it look so. Minor cosmetic changes can be done with gingivoplasty, injectable treatments, or a lip lowering procedure. Correction of large gummy smiles require bone surgery.

DUI Classes - DUI Programs

DUI Classes are different from state to state. In California, a first time offense DUI means that you must complete a 9 month class. You have to be enrolled in this program in order to get a restricted driver's license. Either way, it is a mandatory class that the court orders you to enroll in. An Elegant Company by Pieter Codde

In the galleries of the Art Institute of Chicago is the painting "An Elegant Company" by the 17th Century Dutch master Pieter Codde. This painting depicts a social engagement of an upper class family in Holland.

As so often is the case with Dutch paintings they offer us so much information about the life and times of that era. In this painting we have a gathering of adults for a gracious and elegant gathering. This can be seen by the fine clothing worn by everyone and also the luxurious room with fine appointments and object.

The main figures which dominate the scene are the couple in the center, whom are either the host and hostess or more likely the arrival of very high-ranking persons to the party. The gentleman bowing to the lady in gray is most likely the host of this event and he is expressing his gratitude for their presence with this gesture. The material on the host and the lady to whom he is bowing are made of silk in similar tones. The execution and detail of the paint by Codde here is remarkable for he has captured the texture and the light of the material and so very well placed them on the figures. There is no over-painting or suggestive marks, rather we have very fine and calculated brush strokes to render to us remarkable detail by the artist. The husband or companion of the lady is large and in dark material that too is well rendered but not so demanding on the painterly skills of the artist. The other figure worth noting is the gentleman with a hat on behind the person bowing, he too must be a new arrival for he still has his hat on.

The four figures make up the stage action of the scene and are well supported by the other people in the room. The scene is rather dark considering the light that is coming in from the left (from a window not seen). The light gives highlight to the figures but also casts dark shadows everywhere. To the left near the window is an array of objects from musical instruments to vessels for food and drink. If you look carefully you will see the right arm of a young man reaching for the ewer, the shadows make the actions of this figure difficult to see. There is no sign of any musicians but they must have already played for the group for the musical scores are open and laid around (even on the floor). The attention to detail on the object near the window compliment the same quality of detail in the clothing of the host and lady in the center.

Red Cross CPR Classes Vs American Heart Association CPR Training: Which CPR Certification Is Best?

CPR Certification is something that confuses most people. Which class do you need? Which certification is going to satisfy the requirements for your job?

The two largest CPR training companies are the American Heart Association and the American Red Cross. These companies have been training people to save lives for years and their certifications used to be synonymous. Unfortunately, things have changed and this is not the case anymore.

So what is the difference?

The last thing that you want to do is sit through a four hour CPR or first aid class and realize that you didn't choose the right certification. The most important thing you can do before signing up for a class is to ask your employer or future employer exactly what type of CPR certification you need as well as which certifying agency they are accepting for the position.

The certifying agencies are the American Heart Association (AHA) and the American Red Cross (ARC). Your class choices are as follows:

1.) CPR

2.) CPR and First Aid

3.) Pediatric CPR and First Aid

4.) Basic Life Support for the Healthcare Provider

If you know exactly which class you need then you should have no problem. If you cannot reach your employer, the American Heart Association certification is valid for 2 years whereas the Red Cross cert is only valid for 1 year. The AHA cert is also much more widely accepted in the hospital setting than the ARC. Although both certifications may be valid, the AHA classes will be both more cost effective and more widely recognized. Also, if you work in the medical field and you are not sure which class to take, always opt for the Basic Life Support for the Healthcare Provider. This is the highest level of CPR training and will be accepted everywhere.

So to avoid a CPR nightmare, talk to your boss first! And if you can't do that, stick with a 2 year American Heart Association certification.

How to Stretch Your Silk Painting on a Canvas Frame in 10 Easy Steps

Many silk painting artists and hobby-craft people are constantly looking for new ways to present the works of art that they create. I often get asked if I have any suggestions as to what they could do. And the answer is yes. I'm going to discuss with you in this article how to stretch your silk painting on a canvas frame in a nice straight forward process. So let's get started.

You need to get yourself a chunky artist's canvas in the size you would like to have the painting mounted. Important to note is that the piece of silk you' re going to stretch over it has to be at least 15cm wider and 15cm deeper. Why is that? Because you will need the extra fabric to wrap around the sides of the frame.

So let's get started with the process.

Step 1: Find yourself a clear working space to do this. Place your canvas frame on the table in front of you and carefully lay the silk on top.

Step 2: Gently move the silk around until it is exactly centred on top of the canvas frame. Take time to do this as the more exact you are, the better the final result will be.

Step 3: Fold the silk around the top and bottom and put one or two pins in place to hold the silk firmly.

Step 4: Carefully turn your frame around the other way, with the underside showing up. Now you can be sure that your artwork is nice and centred.

Step 5: Starting with the side facing you, start to staple the silk in place on the wooden frame. Do the centre of the bottom then the top and then swivel your frame around and do left and right.

Step 6: Continue with this process, adding staples halfway along between each of the staples and the corners. Do this for all four sides until the silk is properly attached all the way around the frame.

Step 7: Work each of the corners. Fold the fabric flat in one direction, then tuck the fabric up against it from the opposite direction. Take care to do this neatly and then staple the silk down carefully. Make sure all of your staples are on the underside and cannot be seen from the top when the picture is hanging.

Step 8: Turn your picture around and check that the silk has been attached nice and straight. If you are unhappy with the result, go back and remove the staples and make any adjustments necessary. If you have an abstract design, or anything that isn't symmetrical, then centring the image is not quite so critical.

Step 9: Lay your picture face down again and using a knife and measuring stick, trim off any excess silk. Please only do this when you are wide awake.

Step 10: And now you can hang up your picture. This is a quick and easy way to prepare your silk painting and it has a really contemporary look.

Whether you're out of shape or very fit, spinning classes can help you keep your New Year's resolutions. You ride a stationary bicycle in a group, with a leader who tells you what to do and plays lively music to set the tempo. Many health clubs and gyms offer these classes, and I recommend them to all of my patients. Spinning relatively safe because it is done with a smooth rotary motion that does not involve the muscle damaging road shock of running.

The pedals drive a chain that is attached to a heavy flywheel that spins smoothly as you pedal. The amount of work you do is determined by the resistance on your pedals, not by how fast you spin. You perform a lot of work when you spin slowly against great resistance, and do not do much work when you spin very fast against very little resistance. A brake is attached to the flywheel and you regulate the resistance on the pedals by turning the brake clockwise to increase the resistance, and counter-clockwise to decrease resistance.

Before you start real training, you need to do some background work. Start out by pedaling at a very slow rate with no resistance. Stop when your legs start to feel heavy or hurt. You may be able to go ten minutes on the first day, five on the second, and perhaps not be able to exercise on the third. Persist and eventually every healthy person should be able to work up be able to work up to a full 45-minute class.

Your instructor will have you warm up by spinning without any resistance until you start to break a sweat. Then you will turn up the resistance a little and spin the pedals until your thigh muscles start to burn. Then pedal easily with little or no resistance until you have recovered, usually about a minute or two. You will continue to alternate "into the burn" and "out of the burn" until your leg muscles start to stiffen, which is a signal that you are through for the day.

You need to experiment to learn how high you have to turn up the resistance. If you turn it up too high, you will burn out your muscles and not be able to finish the class. If you don't turn it up high enough, you will not achieve a high level of fitness, not feel sore the next day, and not gain the benefits of being in a high level of fitness. If you have worked out correctly, your thigh muscles will be sore on the next day. Then you have to pedal easily with little or no resistance for as many days as it takes for your muscles to feel fresh again. For most people, this will be one or two days. Then when your muscles are fresh again, you're ready for another hard workout.

Rejuvenation of the Aging Mouth - Corner of Mouth Lift and Nasolabial Fold Augmentation

Age changes around the mouth area are more than just wrinkles and vertical lip lines. The upper lip actually lengthens, the corners of the mouth turn downward and the lips lose fullness. Three distinct grooves appear around the mouth including the nasolabial lines (oblique lip-cheek grooves), marionette lines (vertical chin-facial groove) and the horizontal labiomental groove below the lower lip. These grooves frame the mouth and add to its aging appearance.

Contrary to popular perception, no type of facelift or necklift can change the tissues around the mouth. The pull of these operations is too distant from the mouth to affect the tissues that immediately frame it. The only methods to change the aging mouth must treat it directly.

INDIANAPOLIS

The changes around the mouth with age represent a problem with the thinning and downturning of tissues. Treatments that reverse these problems must, therefore, be about thickening and uplifting these tissues. Direct surgical approaches include thickening with injectable fillers and uplifting through lip lifts and advancements.

The combination of injectable fillers for the nasolabial folds and a direct corner of the mouth lift is an excellent approach to certain aspects of the aging mouth. This combination is particularly beneficial because both procedures affect the same vertical line of tissue aging. Both can be performed as a short office procedure under local anesthesia with no significant downtime.

The use of injectable fillers is a common and very effective method for most deepening nasolabial fold areas. They are not effective when the folds are so deep that they are inverted however. But for shallow and moderate depths folds, injectable fillers will provide a nice softening with an instantaneous result. There are numerous filler options and all will work. I prefer those that last the longest such as Juvaderm, Perlane, Evolence, Radiesse, or Sculptra. Injection under an infraorbital local anesthetic nerve block makes the injection painless.

The corner of the mouth lift is done to turn up the corners of the mouth. This small procedure is quite powerful as it alters the sad mouth appearance. By removing a small triangle of overhanging or downturned skin above each mouth corner, the mouth line becomes level. This does result in a fine line white scar that lies along the upper lip border and extends slightly beyond the mouth corner. This does result in a scar but it is only visible on very close inspection. The little scar is red for a while but will fade to a white color over time. Tiny little dissolveable sutures are used and make-up can be applied immediately.

The combination of nasolabial fold augmentation with injectable fillers and a corner of the mouth lift helps address collateral areas of the aging mouth. In my Indianapolis plastic surgery practice, I perform both under local anesthesia in the office which takes about an hour to complete. These changes are effective and easy to go through with no recovery. Some corner of mouth lifts may require a touch-up but they are very long-lasting. Injectable fillers are temporary and they will need to be repeated every year for their benefits to be maintained.

It is morbid, creepy, morose, and mystifying. The voyage to the great beyond is feared my many and understood by few. The fear of death is so prevalent that it has become the subject of an academic discipline. The study of death anxiety, which is defined as an unpleasant emotional state precipitated by one's own death, has grown over the past four decades creating an outpour of information over the subject. How to Draw the Skeleton of a Frog

Frogs belong to the class 'Amphibia,' characterizing in dual breathing mechanism, skin as well as lungs. The structure of its skeleton is quite simple to draw. The vertical symmetry makes it even easier to construct the details. The following systematic guide will take you through some easy phases to draw your sitting frog skeleton:

• Skull. The skull is the most complex part of the skeleton. Mark a small vertical line. Draw a small circle on the line in such a way that it forms the diameter. You can look at each side as a large-angled arch. The final shape will look like the smaller end of an egg. The 'quadratojugal' extends below the maxillary and continues its arch. Sketch the 'sqamosal' bone as an angular end of the quadratojugal. The 'ethmoid' and 'frontoparietal' complete the central structure.

• Abdomen. The central vertebra is the simplest part of the skeleton of a frog. Place ten vertebral columns on the rib. Of these, keep the above two broader than the rest. The lower end of each of the columns is slightly convex. The lower portion of a rib is a straight bone, known as 'coccyx.' The bones on either side of the coccyx form a blunt 'V-like' shape, which is closed at the upper end by obtuse-angled bones. Terminate the midrib with a pointed bone, the 'pubis-ischium.'

• Limbs. The limbs of a frog primarily have three joints. The frontal limbs are visibly smaller and less angular than the hind limbs. The transverse section of a frog's skeleton features the 'suprascapula,' hiding away the 'scapula' around the joint of the front legs. The upper and lower leg bones are easy to draw. Create the bones of the forwardly placed feet as much smaller, almost half the size, than the backwardly placed ones. Mark 2-4 joints in each of the fingers. Of course, the longest fingers have the largest number of joints. Be extra careful in replicating the exact number of joints as in your 3D model or picture. Draw the almond shaped combination of the 'calcaneum' and the 'astragalus' bones as the upper portion of the hind feet.

The skeletons of animals usually take form of scientific diagrams and do not feature very elaborate shading effects. However, you may make extra pencil marks along the length of the bones and near the joints to bring out the presence of light.

Some say that street art is the most exciting of all graphic art. It has taken several years of wandering the streets of Indianapolis and, when least expecting it, turning a corner and encountering a beautiful piece of art, to appreciate the number and variety of murals in our city. Sharing this experience is the main reason for this article on outside public art which is also in many respects outsider art.

Almost all street art is short-lived, either painted over, destroyed when the building is removed, or simply succumbing to the elements and to deterioration. So documentation becomes another purpose of photographing and writing about mural art.

Since mural art had some of its beginnings in advertising, the first section of the book is about old signs that are still around and ghost signs that are fading but still visible. Some recent signs that are unusual or illustrate an artistic treatment of an advertising theme are also discussed.

The sign at 16th and College advertises a popular soft drink, and a mural at 707 Massachusetts Avenue advertises cereal. Although these murals are very old, these products are still around, and it's interesting to see how their marketing has evolved.

The second very contemporary mural is in Broad Ripple on the Monon Trail, just south of Broad Ripple Avenue. It was painted by Artists Joshua Wells and Dan Thompson of PaintSubsurface in 2006 and advertises a restaurant on the trail using a wave. It is reminiscent of Katsushika Hokusai's famous painting, but this one contains cups, saucers, and sodas. Very cool! Mural styles and influences from other artists and art traditions, is a subject I found most interesting. Muralists apparently build on the work of their predecessors, and their work tends to reflect current artistic trends.

Another example of an old advertising, mural which has recently been beautifully redone, is on Pendleton Pike near Franklin Road and adorns a German restaurant that has been family owned for more than thirty-five years. The restaurant offers a unique cultural dining experience as well as scrumptious baked goods from its bakery, and imported food and gift items that can't be found anywhere else in the city. The newly painted mural is a work of art more than advertising, showing scenes from the German countryside.

A final example of the beginnings of mural art in advertising can be found on Indiana Ave., now Martin Luther King Boulevard near 26th street. Piggy speaks for himself on this restaurant saying, "Best in the World since 1962". This mural is near the Indiana Avenue Cultural District which was the home of jazz in Indianapolis for many decades and still boasts the Madame Walker Theater.

The community mural movement, which started in the late 1960's, produced hundreds upon hundreds of large scale wall paintings in less than a decade. They were concentrated in major cities, but almost every city had a few, including Indianapolis. These were sparked by the civil rights movement, labor movements, the Chicano movement, and community development programs. Hispanic muralists such as Diego Rivera, Clemente Orozco, David Sequeiros, and Arnold Belkin influenced the style and content of these murals. They were also influenced by graffiti and by advertising art.

Murals with a message describes some ambitious community murals such as, "The Street of Dreams". It is located in the Watkins Park Family Center and includes portraits of many of the well known jazz artists in the city such as David Baker, Cheryl Hayes, and Jimmy Coe. The equally talented muralists who painted this and many other murals relating to the community mural movement are Barbara Stahl, Carol Tharp-Perrin, Larry Ginhardt and Lawrence Clark IIII. In Indianapolis, many of these murals are gone, but a few can still be seen in city parks. A mural in Douglass Park on the near east side celebrates one of the foremost leaders of the abolitionist movement, which fought to end slavery in the United States before the Civil War.

An interesting and unique example of mural art is still visible near 38th and College. Clayton Hamilton's Message Wall has posted community related messages and political commentary for years. He changes the wall frequently. Some examples include, "Vote out Greed, Lies, and Secrecy" and "Our Vote Mattered...Power to the People in '06 and '08!"

A mural commemorating the U.S.S. Indianapolis by Cynthia Walden, painted in 2002, is located under the New York street bridge over the canal in downtown Indianapolis and shows the ship superimposed on an American Flag with the caption "Freedom isn't Free.

The final section of this article deals with murals in churches and schools and in various neighborhoods of the city. It covers outside murals only, as their are numerous inside murals in public schools and in many places of worship.

An unusual and unsolicited work of mural art can be seen at east 10th and Sherman Drive beneath the underpass. It was painted by Tami Lannom and intended to beautify the neighborhood and inspire with its message, "Whatever you do may seem insignificant, but it is most important that you do it." Her message seemed to encourage many other artists in this area, resulting in the painting of other exceptional wall art. With the attention to development being paid to this neighborhood because of the city's hosting of the Superbowl in 2012, many more can be expected. The other neighborhoods in the city can boast similar mural achievements.

There are also numerous murals on the Monon Trail and in the city's newly designated cultural districts including, Broad Ripple, Indiana Avenue and Massachusetts Avenue, Fountain Square, Downtown and Wholesale District, and the Canal District. Street art and graffiti, Eli Lilly & Company's Global Service Day murals, and new murals planned for sprucing up the city for the Super Bowl in 2012 add to the mix.
